I, the undersigned, hereby release Pretty Heart Inc., its employees, volunteers, and agents, from any and all liability, claims, or demands arising out of or in connection with the use of my or my child's photograph, likeness, name, or other personal information for any purpose, including but not limited to publicity, advertising, and promotion.

 

I understand that Pretty Heart Inc. intends to use this information for recognition and reward purposes only, and that it will not be sold or distributed to any third parties. However, I acknowledge that Pretty Heart Inc. cannot guarantee the security of this information and cannot be held liable for any unauthorized use or misuse by third parties.

 

I hereby waive any right to inspect or approve the finished photographs or other materials in which my or my child's image or information may appear. I also waive any right to royalties or other compensation arising from the use of these materials.

 

I understand that this release is binding on me and my heirs, executors, and assigns.
